Life Care Planning

 A Life Care Plan is a dynamic document based on published standards of practice, comprehensive assessment, data analysis and research which provides an organized, concise plan for current and future needs, with associated costs, for individuals who have experienced catastrophic injury or have chronic health care needs. At Feerick Medical Planning, we use a comprehensive approach to develop a Life Care Plan that clearly demonstrates the client's current medical status and future plan of care. The Life Care Plan will include recommendations based on peer-reviewed literature and current practice that will allow for maximum functional ability and optimum outcome achievement for the injured individual.

Feerick Medical Planning will meet with the client, if authorized, review and summarize all medical records, and build the Life Care Plan based on the specific nature of his or her injury, lifestyle and health care needs.  Our reports are concise and clear and based entirely on the client’s healthcare and support needs.

Feerick Medical Planning will clearly note all associated costs in the Life Care Plan. The costs are based on community standard coupled with current research. The summary, recommendations and costs are cohesive and produce a tool that can be used as a basis to manage the injured person’s long term medical care.

Feerick Medical Planning also provides services such as cost analysis, life care plan critique, medical costs estimates and medical record review and summary.
